Our Resident Services Program

The Bay City Housing Commission (BCHC) is committed to assisting its residents access supportive services.  We know from experience that many housing programs are most effective when coupled with a robust integrated supportive service model. The BCHC has restructured its services platform so that it provides direct support to all its families. Residents will have access to personalized services and referrals from a dedicated staff of property managers and social services case managers. The Resident Services Program (RSP) is designed as a resource to assure residents gain access to community resources that enhance their lives.

The Great Lakes Bay Health Center

Great Lakes Bay Health Centers Mobile Outreach Unit is available to all Bay City Housing Commission and neighborhood residents.  Please call 989-907-2761 to schedule an appointment.  Mobile Outreach is stationed at the parking lot of the Bay City Housing Commission, 315 14th Street (cross street Garfield Avenue) usually during the third week of each month. 

Great Lakes Bay Heath Care Mobile Services include:

Primary Care Services
– Physical Health Assessments
– Acute Visits (cough, cold, sore throat, ear ache, etc.)
– Chronic Care (Diabetes, Hypertension, Asthma, etc.)

Behavioral Health Services
– Psychiatric Evaluations
– Psychiatric medication reviews
– Brief behavioral health interventions (Anxiety, Depression, PTSD, coping skills, etc.)

Medication Assisted Treatment for Substance Use Disorders
– Vivitrol** / Oral Naltrexone
**Proof of participation in individual therapy sessions required for Vivitrol Program.

Most insurances accepted for services. Sliding fee scale available if individual is uninsured.
